Qualigenix medical coding services support independent practices across Indianapolis and Marion County that bill a mix of Indiana Medicaid plans, Medicare and commercial insurers. The rules that apply statewide, such as IHCP code sets and fee-schedule prior authorization flags, are on our medical coding services in Indiana page. This page covers the coding decisions that change by payer for a Marion County practice. Qualigenix codes for all payers. Your coders are matched to your specialty, work inside your current EHR and feed denial patterns back into their coding so the same error doesn't repeat.
Qualigenix codes professional and outpatient encounters for Indianapolis practices with CPC- and CCS-certified coders.
We read the documentation, assign ICD-10-CM, CPT and HCPCS Level II codes, apply only the modifiers the record supports, check medical necessity against WPS coverage policy, query providers when notes fall short and run two levels of QA on every chart. Coders work inside your EHR.
Qualigenix codes each Indiana Medicaid encounter to the policies of the plan that covers the member. IHCP sets coding and prior authorization policy for Traditional Medicaid, but each managed care entity (MCE) sets and publishes its own prior authorization, billing and reimbursement criteria. The same CPT code can pay under one plan and deny under another.
Indianapolis practices code for HIP and Hoosier Healthwise members in Anthem, CareSource or Managed Health Services (MHS) since MDwise left both programs on January 1, 2026. Hoosier Care Connect runs through Anthem, MHS and UnitedHealthcare, and PathWays for Aging through Anthem, Humana and UnitedHealthcare. We keep a policy file for each plan, so coders know which procedure codes need a prior authorization number on the claim and which plan edits bundle common code pairs. Traditional Medicaid changes count too: IHCP removed prior authorization from audiology codes 92607 and 92608 for dates of service from January 10, 2026.
The FY 2027 ICD-10-CM code set took effect for encounters on and after October 1, 2026. Codes deleted that day now reject if they stay in a template, a superbill or a provider's favorites list. Our coders updated each client's code lists before October 1 and recheck specialty templates when a plan rejects a diagnosis as invalid.
IHCP fee schedule flags, Traditional Medicaid code-level limits and statewide audit programs are covered on our Indiana medical coding rules page.
Qualigenix codes Traditional Medicare claims to the local coverage determinations and billing articles published by Wisconsin Physicians Service (WPS), the Medicare contractor for Indiana and Michigan. When a WPS policy limits which diagnoses support a test or procedure, coders check the note for those diagnoses before the claim goes to medical billing. Medicare Advantage plans, including the PathWays Dual Care plans that launched in January 2026, follow their own policies, and we code to each. For commercial plans, Indiana requires utilization review entities to post their current prior authorization requirements and written clinical criteria on their websites. Coders check those posted criteria before coding a procedure that needs approval, so the codes on the claim match what the payer approved.
These are company-wide results across Qualigenix client practices, not Indianapolis-only figures. Each measure is defined in your monthly coding report, so you can follow the same number for your own charts.

Yes. We code for Traditional Medicaid and for every managed care plan, including Anthem, CareSource, MHS, UnitedHealthcare and Humana. Each plan sets its own prior authorization and reimbursement criteria, so coders check the plan's policy file before coding procedures it restricts.
Coders check each test and procedure against the WPS local coverage determination or billing article that applies, then confirm the note documents a covered diagnosis. If it doesn't, they query the provider before the claim goes out instead of sending a claim that will deny.
Yes. Qualigenix coders hold CPC and CCS certifications and are assigned by specialty. Every chart they code passes two levels of QA, and accuracy is scored by coder and specialty. Ask a coding specialist which coders would be assigned to your practice.
The FY 2027 code set applies to encounters on and after October 1, 2026. Our coders updated client code lists before that date, removed deleted codes from templates and track any rejections that point to an outdated diagnosis code.
No. Coding accuracy depends on certified coders who know your specialty, WPS coverage policy and each Indiana Medicaid plan's rules. Qualigenix coders work inside your existing EHR, and your monthly report shows accuracy by coder, so you can measure the work directly.
Qualigenix charges a percentage of collections. The percentage depends on your specialty, chart volume and encounter types, so we set it after a short review of your practice. A coding specialist can explain how it would apply to your charts.
