Qualigenix medical billing services start with the payer mix your front desk already deals with. A Richmond practice usually sees Cardinal Care members spread across several health plans, traditional Medicare and Medicare Advantage patients, dual-eligible patients in aligned plans, and commercial members under both Virginia-regulated and self-funded plans. Each group has its own filing window, edit set and appeal path. We bill every one of them from inside your current EHR or practice management system, so your staff keeps its workflow and stops chasing payers. Statewide filing deadlines and prompt-pay rules are on our medical billing services in Virginia page. The sections below cover what changes when the patient is a Richmond Cardinal Care member, a Palmetto GBA Medicare beneficiary or a member of a Virginia commercial plan.
Qualigenix handles the full claim cycle for Richmond practices: eligibility checks, charge review, claim scrubbing, submission, payment posting, denial appeals and A/R follow-up.
We bill all payers, including Virginia's five Cardinal Care health plans, Medicare through Palmetto GBA and commercial carriers. You pay a percentage of collections, so our fee moves with what your practice actually collects.
Qualigenix sends each Cardinal Care claim to the health plan the member is assigned to on the date of service, not the plan printed on an old card. Most Virginia Medicaid members are in managed care, so plan assignment decides where the claim goes, which edits apply and how long you have to file.
Since July 1, 2025, Cardinal Care Managed Care has run through five health plans: Aetna Better Health of Virginia, Anthem HealthKeepers Plus, Humana Healthy Horizons in Virginia, Sentara Health Plans and UnitedHealthcare Community Plan. Molina left the program that day, and members who hadn't picked a new plan were moved to Humana. A practice that still has Molina IDs in its registration screens sends claims to a payer that no longer covers the patient. Qualigenix rechecks plan assignment through the DMAS provider portal before each claim goes out and routes it to the plan that covers that date.
Each health plan sets its own filing limit. Anthem HealthKeepers Plus, for example, gives 365 days from the date of service for original claims and 12 months from the payment explanation for corrected claims. Qualigenix loads each plan's limits into the claim queue and works anything close to its limit first, so a Richmond claim doesn't age out while it waits on a rebill.
Some Cardinal Care members stay in fee-for-service, and DMAS expects those claims within 12 months of the date of service. When Medicare paid first, DMAS can pay the crossover claim within six months of the Medicare decision. Every rendering provider must also be enrolled in the DMAS provider portal, called PRSS, or neither DMAS nor the health plans can pay. We confirm enrollment before your first claim goes out.
Qualigenix bills traditional Medicare for Richmond practices through Palmetto GBA, the Jurisdiction M contractor for Virginia. Part B claims for Arlington and Fairfax counties and the city of Alexandria go to Novitas instead, which matters for groups with a Northern Virginia location. Since January 1, 2025, a full-benefit dual-eligible patient who picks a D-SNP gets Cardinal Care coverage from the same company, and DMAS bars providers from billing those members for Medicare cost sharing. Our patient statements leave those balances off. Commercial carriers regulated in Virginia must pay a clean claim within 40 days and flag any defect within 30 days, and those notices have to arrive electronically since January 1, 2026. We log the received date on every commercial claim and follow up the day a payment runs late.
These are company-wide results across Qualigenix client practices, not Richmond-only figures. They show what our billing process produces once your claims run through it.

Qualigenix moves a Richmond practice onto our billing team in six steps, with a 6-day average from kickoff to live claims.
We sign the BAA, assign a named onboarding owner and connect to your EHR or practice management system with no software change. We also pull a current A/R snapshot so nothing in flight gets dropped.
We confirm each payer enrollment, payer ID and effective date, including your PRSS record and each Cardinal Care plan contract.
Specialty coders are matched to your services, and claim edits are set for your highest-volume payers.
We move clearinghouse, ERA and EFT enrollment to Qualigenix and run a test transaction before any live claim.
The first batch is coded, scrubbed and checked against expected payment. Any payer that isn't effective yet goes on hold until it is.
Live claims go out daily, and your legacy A/R is worked in parallel so older money keeps coming in.
Certified coders and billers, direct EHR operation, defined metric reporting, and seamless six-day onboarding.
Specialty-certified coders and dedicated billers handle your claims, so the person working a denial knows your codes and the plan that denied them. You get named contacts, not a rotating queue.
We work across 133 EMR and EHR platforms. Your staff keeps its system and its routine, and patient data stays where it lives today.
Monthly reports show clean-claim rate, denials, collections and A/R by payer. Every metric comes with its definition, so you can check the math yourself.
Most practices go live within a week. Legacy A/R is worked alongside new claims, so cash doesn't stall during the switch.
Frequently asked questions regarding Richmond Cardinal Care plans, PRSS provider enrollment, Palmetto GBA Medicare processing, and commercial prompt-pay rules in Richmond, VA.
No. What matters is knowing Richmond's payers: the five Cardinal Care health plans, Palmetto GBA for Medicare and Virginia's commercial claim rules. Qualigenix bills all of them from inside your existing system. Talk to a billing specialist to see how that would work for your practice.
All five: Aetna Better Health of Virginia, Anthem HealthKeepers Plus, Humana Healthy Horizons in Virginia, Sentara Health Plans and UnitedHealthcare Community Plan. Qualigenix also bills fee-for-service Medicaid to DMAS and every other payer your practice sees.
Fee-for-service claims are due to DMAS within 12 months of the date of service. Each Cardinal Care health plan sets its own limit, so Qualigenix tracks every claim against its plan's deadline and works the oldest first.
Palmetto GBA processes Medicare Part A and Part B claims for Richmond as the Jurisdiction M contractor. Qualigenix builds Richmond Medicare claims to Palmetto GBA's edits and coverage policies before they go out.
Carriers regulated in Virginia must pay a clean claim within 40 days of receiving it and must notify you of any defect within 30 days. Qualigenix records the received date on every claim and follows up when a payment runs late.
Qualigenix charges a percentage of collections, so the fee moves with what your practice collects. A billing specialist can walk you through how it applies to your practice on a short call.
Yes. Qualigenix signs a business associate agreement before accessing any patient data and works inside your existing EHR, so records stay in your system.
