Qualigenix medical billing services start with your payer mix. A Warren practice usually bills several Medicaid Health Plans, traditional Medicare, Medicare Advantage, dual-eligible plans and commercial insurance in the same week, and each one pays on its own rules. Statewide rules, such as MDHHS's 12-month Medicaid filing limit and Michigan's clean-claim timelines, are covered on our medical billing services in Michigan page. This page covers what changes for a practice in Warren and the rest of Macomb County: which plans you'll bill, where behavioral health claims go, and how dual-eligible patients are covered in 2026.
Qualigenix runs the full claim flow for Warren practices: eligibility checks, charge and coding review, claim scrubbing, submission, payment posting, denial appeals and A/R follow-up. We bill all payers, including the Macomb County Medicaid Health Plans,
MI Coordinated Health plans, Medicare through WPS and commercial plans. Pricing is a percentage of collections.
Most Medicaid patients in Warren are enrolled in a Medicaid Health Plan, not fee-for-service Medicaid. Qualigenix sets up each plan as its own payer, with its own payer ID, filing limit and authorization list. Coverage will move faster in 2027. From January 1, 2027, most Healthy Michigan Plan adults renew every six months instead of once a year, and some adults 19 to 64 must meet new work requirements. A patient covered in March may not be covered in September, so we check eligibility before every Medicaid visit, not just the first one.
Macomb County sits in Michigan's Detroit Metro Prosperity Region, which MDHHS contracted to eight Medicaid Health Plans starting October 1, 2024. Plans serving the county include Aetna Better Health of Michigan, HAP CareSource, McLaren Health Plan, Meridian, Molina Healthcare of Michigan, Priority Health and UnitedHealthcare Community Plan. Qualigenix checks which plan a patient is assigned to before the visit, because the card a patient brings isn't always current. Claims go to the assigned plan the first time, which keeps wrong-payer denials off your A/R report.
Michigan carves specialty behavioral health out of the Medicaid Health Plans. In Macomb County, those services are managed by Macomb County Community Mental Health (MCCMH), and MDHHS kept the current PIHP and CMH structure after withdrawing its rebid in February 2026. Qualigenix sorts each behavioral health claim by service type, so mild-to-moderate care goes to the patient's health plan and specialty services go through MCCMH's process.
Timely filing, replacement claims and retroactive eligibility follow the same MDHHS rules across Michigan. Our page on how Qualigenix handles Michigan Medicaid claims statewide explains each one.
Traditional Medicare claims from Warren go to WPS Government Health Administrators, the Jurisdiction 8 Medicare contractor for Michigan and Indiana. Qualigenix sets up your WPS EDI enrollment and scrubs each claim against Medicare edits before it's sent. Dual-eligible billing changed on January 1, 2026. MI Coordinated Health replaced MI Health Link, and Macomb is one of the first counties where it's offered. Eight plans offer it in the county for 2026: Aetna, AmeriHealth, HAP CareSource, Humana, Molina, Priority, UnitedHealthcare and Wellcare-Meridian. Each one covers Medicare and Medicaid benefits under a single plan, so Qualigenix bills one claim to that plan instead of a Medicare claim followed by a Medicaid crossover. Warren practices also bill Medicare Advantage plans, which set their own authorization lists and filing limits. Since 2026, Medicare Advantage plans must decide standard prior authorization requests within 7 calendar days and urgent ones within 72 hours. We log each request date so a late decision gets escalated, not forgotten. Commercial claims, including Blue Cross Blue Shield of Michigan and other plans your patients carry, are billed to each contract's filing window. Contracts often set shorter limits than Michigan's one-year clean-claim rule, so we follow the contract date, not the state minimum.
These are company-wide results across Qualigenix client practices, not Warren-only figures.
