Qualigenix medical billing services cover independent practices in Maryland across 38+ specialties, and we bill all payers. Medicaid alone brings a lot of rules. Maryland's nine HealthChoice managed care organizations cover nearly 1.3 million residents, and each MCO runs its own portal, claim edits and filing window. Add Novitas for Medicare and the commercial carriers, and one practice can bill a dozen payers with different rules. Qualigenix builds a rule set for every payer on your account before the first claim goes out. The process is the same whether your practice sits in Baltimore City, Montgomery County, Frederick or on the Eastern Shore. Specialty behavioral health claims for Medicaid patients don't go to the patient's MCO. Carelon Behavioral Health has processed them for the state's public behavioral health system since January 1, 2025, so our billers route those claims separately. Practices that want eligibility, authorizations and collections under one team can use our revenue cycle management services in Maryland.
Qualigenix runs the full claim cycle for Maryland practices: eligibility checks, charge review, claim scrubbing and submission, payment posting, denial appeals and A/R follow-up. Claims go to Maryland Medicaid fee-for-service, the HealthChoice managed care plans, Novitas Solutions for Medicare and every commercial payer.
Your team keeps seeing patients while our billers work the claims inside your existing EHR.
Most lost revenue in Maryland billing comes from four places: Medicaid deadlines, slow commercial payment, Medicare coverage edits and patient statements sent to people who shouldn't get them. Qualigenix sets up a workflow for each one before your first claim goes out.
Maryland Medicaid expects a clean fee-for-service claim within 12 months of the date of service. A corrected claim after a denial must also land inside that same 12 months, so the clock doesn't restart when a claim comes back. Medicare crossover balances are due within 120 days of the Medicare payment notice. Each HealthChoice MCO sets its own filing limit, and several are shorter than the state's. Qualigenix tracks every Medicaid claim by date of service and by plan, then works rejections while there's still time to refile. Our denial management team keeps a separate queue for claims nearing a deadline.
Maryland requires commercial carriers to give providers at least 180 days from the date of service to submit a claim. A carrier must pay a clean claim within 30 days or tell you what's missing. If it pays late, it owes monthly interest: 1.5% from day 31 to 60, 2% from day 61 to 120, and 2.5% after that. Qualigenix flags every commercial claim unpaid at day 30 and checks the remittance for the interest owed. Old balances go to our A/R recovery specialists, sorted by payer and age.
Novitas Solutions is the Medicare administrative contractor for Maryland, part of Jurisdiction L with Delaware, the District of Columbia, New Jersey and Pennsylvania. Medicare gives you 12 months from the date of service to file. Novitas also publishes local coverage determinations that decide which diagnoses support which procedures. Before a Medicare claim leaves, our medical coding team checks the diagnosis against the Novitas policy for that service. For dual-eligible patients, we confirm the claim crosses over to Maryland Medicaid.
Maryland Medicaid patients can't be billed for covered services. Members of HMOs regulated under Maryland law can't be balance billed for covered care either. In Maryland-regulated PPO and EPO plans, on-call and hospital-based physicians who accept assignment of benefits can't balance bill the patient. The federal No Surprises Act adds its own limits. Qualigenix checks the plan type and network status before any statement goes out, so a protected patient never gets a bill they don't owe.
These are company-wide results across Qualigenix client practices, not Maryland-only figures. Our medical coding services in Maryland feed the clean-claim and denial numbers below.

Qualigenix brings a Maryland practice live in six steps, with a 6-day average onboarding and no software changes.
We sign a BAA, name an onboarding owner and connect to your EHR or PM system. You don't change software. We pull an A/R snapshot so nothing in progress gets dropped.
We confirm your payer IDs and effective dates with Maryland Medicaid in MPRIME, Novitas and each commercial plan. Gaps go to our credentialing services in Maryland team.
Specialty-certified coders matched to your specialty mix join the account. We set claim edits for your highest-volume payers, starting with the HealthChoice MCOs you see most.
We connect your clearinghouse and route ERAs and EFTs to us, payer by payer. A test transaction runs before any real claim goes out.
The first batch gets coded, scrubbed and checked against expected payment. We hold claims for any payer whose enrollment isn't effective yet.
Live claims go out daily. Legacy A/R gets worked in parallel, and your monthly reporting schedule starts.
Specialty-certified coders, dedicated billers, seamless operation across 133 EHRs, and deep expertise in HealthChoice MCOs, Novitas Solutions, and MPRIME.
Your account gets coders certified in your specialty and billers who know your payers by name. The same people work your claims every month, so you don't have to retrain anyone.
Qualigenix works inside 133 EMR and EHR platforms. We use your existing system and logins. Your front desk keeps its current workflow, and nothing moves to a separate portal.
Each month you get clean-claim rate, denial rate, days in A/R and collections by payer. Every metric comes with its definition, so you know exactly what the number measures.
Most practices go live in 6 days on average. Old A/R gets worked while new claims go out, so cash keeps coming in during the switch.
