Qualigenix provides medical billing services to independent practices across Indiana, and we bill every payer your patients carry. For most Indiana offices, that mix starts with IHCP. Its four managed care programs, the Healthy Indiana Plan (HIP), Hoosier Healthwise, Hoosier Care Connect and Indiana PathWays for Aging, cover more than 1.4 million Hoosiers. Traditional Medicare, Medicare Advantage and employer plans make up the rest. Each payer type runs on its own clock. IHCP fee-for-service claims, managed care claims, Medicare claims and commercial claims each carry a different filing window and payment timeline. Qualigenix loads those rules into your account's claim scrubber before the first claim goes out, so a claim at risk of missing its window gets flagged while there's still time to fix it. Billing sits inside our wider revenue cycle management work, so coding, posting and A/R stay with one team. Qualigenix also offers medical coding services in Indiana, revenue cycle management services in Indiana and provider credentialing services in Indiana.
Qualigenix runs the full claim cycle for Indiana practices: eligibility checks, coding review, claim scrubbing, submission to Indiana Health Coverage Programs (IHCP) plans, Medicare and commercial payers, payment posting,
denial appeals and A/R follow-up. We bill all payers, and every claim is tracked against its payer's filing deadline.
Indiana practices lose revenue in four places: Medicaid claims filed late or sent to the wrong plan, commercial claims left unpaid past the state's prompt-pay window, Medicare claims that fail contractor edits, and patient bills that miss estimate and notice rules. Here's how Qualigenix handles each one.
IHCP gives fee-for-service claims 180 days from the date of service, and its managed care plans hold in-network providers to 90 days. Since January 1, 2026, MDwise no longer serves HIP or Hoosier Healthwise members, so those patients moved to Anthem, CareSource or MHS. Qualigenix confirms the member's current plan at every visit through eligibility verification, sends the claim to that plan and tracks it by date of service. When IHCP grants eligibility retroactively, we file with the proof needed to extend the window.
Indiana requires insurers to pay or deny a clean claim within 30 days when it's filed electronically and 45 days on paper, and to report any defect inside the same window. When payment comes late, interest starts on day 31 or day 46. Qualigenix files electronically, logs each payer's acknowledgment and works every claim still open on day 31 through A/R recovery. Indiana also gives insurers two years to request an overpayment back, and we track each recoupment request against that date.
Wisconsin Physicians Service (WPS) is the Medicare administrative contractor for Indiana under Jurisdiction 8, which also covers Michigan. Qualigenix builds Part B claims to WPS edits and local coverage policies and files well inside Medicare's 12-month limit. For patients with both Medicare and IHCP coverage, we follow the claim through crossover to the patient's Medicaid plan. When WPS denies a line, our team corrects it or opens a redetermination through denial management.
Indiana patients can ask for a good faith estimate before scheduled, non-emergency care. An out-of-network practitioner working in an in-network facility must give written notice, an estimate and a consent form at least five days ahead before charging more than in-network rates. Qualigenix bills patients only for the balance their plan's explanation of benefits supports, checks each statement against any estimate on file and writes payment plan terms in plain language.
The figures below are company-wide results across Qualigenix client practices, not Indiana-only numbers. They show what changes when billing moves to a team that works claims every day.
