Qualigenix provides provider credentialing services for solo providers, independent practices and groups in Louisville and across Jefferson County. Enrollment order matters here. A provider needs an active Kentucky Medicaid ID before the Medicaid plans will credential them, and in Jefferson County the plan contract that reaches the most patients is Passport by Molina, which held 109,659 of the county's 225,170 Medicaid members in July 2026. Statewide enrollment steps, Medicare enrollment and licensing rules are on our credentialing services in Kentucky page. This page covers the Louisville view. We credential with every payer type: Medicaid, Medicaid managed care, Medicare, Medicare Advantage, dual-eligible plans and commercial insurers.
Qualigenix handles credentialing and payer enrollment for Louisville providers and groups. We build and maintain CAQH profiles, enroll providers in Kentucky Medicaid, credential them with the Medicaid managed care plans, enroll them in Medicare through PECOS and submit commercial applications. Then we follow up with each payer and track revalidation dates. Pricing is $99 per payer application. Payers make the final approval decision.
Each payer application costs $99. Payers make the final decision, and we report status on every application until it closes.
Qualigenix sequences Louisville Medicaid credentialing so state enrollment, plan credentialing and plan contracts move in the right order.
Every Kentucky Medicaid enrollment, revalidation and update now runs through the state's Medicaid Partner Portal Application, and paper forms aren't accepted. Providers revalidate at least every five years. In June 2026, Kentucky Medicaid also told providers that CMS directed it to run off-cycle revalidations over 24 months, so some providers must revalidate even if they did so in the last four years. Qualigenix watches the portal for those notices and files on time. Two portal details trip up practices: applications left in progress with no activity for 120 days are withdrawn automatically, and some provider types owe an application fee unless they already paid one to Medicare or another state. We keep files moving and attach proof of any fee already paid.
Each of the five Kentucky Medicaid managed care plans credentials and contracts with providers separately. Aetna Better Health of Kentucky, for example, credentials through the Kentucky Health Alliance, which uses Verisys for primary source verification. Passport by Molina asks providers to authorize the plan to view their CAQH profile. Qualigenix prepares each plan's packet, authorizes CAQH access for every plan you join and starts with Passport for most Louisville practices given its share of county members.
Kentucky Medicaid enrollment steps, Medicare enrollment and state licensing checks are covered on our Kentucky credentialing rules page. Louisville providers follow those steps with the plan sequencing above.
Qualigenix enrolls Louisville providers in Medicare through PECOS, with applications processed by CGS Administrators as the Jurisdiction 15 contractor for Kentucky. Medicare Advantage and dual-eligible plans contract separately. In Jefferson County, 2027 dual-eligible special needs plans come from Humana, Aetna, UnitedHealthcare, Wellcare and Passport Advantage, and each needs its own network application. For commercial managed care plans, Kentucky requires insurers to decide on a credentialing application within 45 days once it contains everything the current CAQH form asks for. That puts the weight on a complete, current CAQH profile. Qualigenix re-attests CAQH on a 90-day cadence, ahead of CAQH's 120-day requirement, and verifies licenses with the Kentucky Board of Medical Licensure or the Kentucky Board of Nursing for APRNs.
Qualigenix reports credentialing status per payer for every client. These are company-wide figures, not Louisville-only results, and payers make the final approval decision.

We gather NPI, Kentucky license, DEA, board certifications, CV, work history and malpractice coverage, then set up or update each provider's profile.
Licenses, board certification, DEA registration, education and malpractice history get verified at the source to NCQA and CMS standards.
We build or clean up each CAQH ProView profile, attest it and authorize access for every payer you plan to join.
