Dialysis revenue is lost in the rules around the treatment: a bundled drug or lab billed outside the ESRD PPS, a monthly capitation claim billed at a visit level the notes don't support, or Medicare billed as primary during a patient's 30-month coordination period. Qualigenix bills dialysis facility and nephrology claims so the bundle, the visit count and the payer order on each claim are right.
Medicare pays ESRD facilities a single bundled rate per treatment under the ESRD Prospective Payment System, covering dialysis plus ESRD-related drugs, labs and supplies, with adjustments for patient case-mix. Facilities bill monthly on type of bill 72x, while nephrologists bill a monthly capitation payment based on the patient's age and the number of face-to-face visits. Qualigenix bills in-center, home and hospital-based dialysis programs and nephrology practices, with a 98.6% compliance and audit pass rate. The ESRD PPS rate is adjusted for patient factors such as age, body size, comorbidities and the first months of dialysis, plus outlier and add-on payments. Since January 1, 2025, oral-only drugs such as phosphate binders are part of the bundle. A missed adjuster underpays the claim, and an ESRD-related drug or lab billed outside the bundle is an overpayment.
Commercial and Medicare Advantage plans often require prior authorization for home dialysis equipment, specialty drugs, vascular access procedures and transplant evaluations. Treatment that starts before the approval often goes unpaid.
ESRD facilities bill one claim per patient per month on type of bill 72x, with a line for each treatment. Condition codes tell Medicare the setting: 73 for self-care training, 74 for home dialysis and 76 for back-up dialysis in the facility. A wrong condition code sends the claim down the wrong payment path.
Home hemodialysis and peritoneal dialysis are billed differently from in-center care, and training sessions are billed with their own condition code. When a patient starts home training within the first three months of dialysis, Medicare entitlement can begin in the first month instead of the fourth.
When a patient with ESRD has employer group health plan coverage, that plan pays first for a 30-month coordination period, and Medicare pays second. Billing Medicare as primary during that window creates an overpayment that is recouped later. Entitlement also depends on a filed CMS-2728 form.
Every treatment needs a current dialysis order with the prescription, and peritoneal dialysis orders need exchange frequency and dwell time. Medicare pays three hemodialysis treatments a week by default, and extra treatments need documented medical justification under the contractor's frequency policy.
Dialysis denials and recoupments usually trace to MSP errors, missing or expired treatment orders, extra treatments without justification, MCP visits not documented, or labs and drugs billed outside the bundle by another provider.
Under consolidated billing, ESRD-related labs and drugs are paid through the facility's bundled rate, not billed separately by outside labs or pharmacies. Services unrelated to ESRD can be billed outside the bundle with modifier AY. Medicare's ESRD Quality Incentive Program can also cut a facility's payment by up to 2% for missed quality targets.
Qualigenix closes each gap with a check built into the monthly dialysis billing cycle.
We capture every case-mix adjuster the record supports and keep bundled drugs, labs and supplies on the bundled claim. Qualigenix holds 98.9% coding accuracy and an over/undercoding rate under 2%.
We submit and track authorizations for home dialysis equipment, specialty drugs and access procedures, and hold claims until approvals are on file.
We build each 72x claim with the right revenue codes, condition codes and treatment lines, and reconcile treatment counts against the flowsheet before the claim goes out.
We set up home and training claims with condition codes 73 and 74, track training sessions, and flag patients whose early home training changes their Medicare start date.
We confirm each patient's CMS-2728 filing, Medicare entitlement date and any employer coverage, and track the 30-month coordination period so the right payer is billed first.
We check that every treatment has a current order and that extra hemodialysis treatments have documented justification before they are billed.
We work dialysis denials and recoupments by root cause and appeal with the missing order, entitlement record or visit documentation. Across our book of business, 79% of appealed denials are overturned.
We watch for ESRD-related labs and drugs billed outside the bundle and apply modifier AY only to services that are not related to ESRD treatment.
A 3-provider podiatry group with roughly 60% Medicare payer mix carried $412,000 in A/R, 38% of it past 120 days. Routine foot care claims (11720, 11721, 11055–11057) had stalled on inconsistent Q7/Q8/Q9 modifiers and class-finding documentation, so payers held or partially paid and nobody worked the remainder. Medicare crossover to secondaries was not being posted, and small patient balances were written off.
We ran a full A/R aging segmentation, worked claims by payer and denial reason rather than by date, rebilled the stalled routine-care claims with corrected modifiers, and reactivated secondary and crossover billing.
A solo podiatrist coding 320 encounters a month was stuck at 82% first-pass acceptance. Routine foot care went out without the systemic-condition diagnosis pairing and class findings Medicare requires, so it read as non-covered. Wound debridement was coded to the wrong depth (11042 for skin and subcutaneous where the note supported 11043 for muscle and fascia). Same-day E/M with a procedure went out without modifier 25 and got bundled.
We rebuilt the coding workflow: diagnosis-to-CPT pairing rules for at-risk foot care, correct Q-modifier logic, depth-based debridement coding tied to the operative note, and disciplined use of modifiers 25 and 59/XS.
A 2-location podiatry practice sat at a 19% denial rate, mostly Medicare routine-foot-care and nail-debridement rejections. The denials clustered in three buckets: medical-necessity rejections on routine foot care (missing qualifying diagnosis and class findings), frequency-edit denials on nail debridement billed inside the 60-day window, and missing Q modifiers that flagged services as non-covered.
We built a front-end denial-prevention layer, checking eligibility and coverage against payer LCDs before claims went out, plus a structured appeal workflow with documentation templates mapped to each denial reason.
Qualigenix bills the full dialysis service mix, from in-center treatments to nephrologist monthly capitation.
In-center hemodialysis is billed under revenue code 0821, one line per treatment, on the monthly 72x claim.
Qualigenix bills for in-center dialysis facilities, home dialysis programs, nephrology-affiliated dialysis units, hospital-based dialysis centers and multi-site dialysis organizations.

Our dialysis billers work the ESRD PPS, MSP rules and monthly capitation codes every day, so facility and nephrology claims hold up on review. CMS's own error data shows how often nephrology claims miss. 16.6% improper payment rate 51.8% from documentation gaps 23.2% from coding errors $212.5M paid in error Source: CMS CERT, 2025 Medicare FFS Supplemental Improper Payment Data, Tables I1 and J1 (Nephrology, Part B; claims submitted July 2023 to June 2024).
Qualigenix charges a percentage of collections, so the fee moves with what your facility is actually paid. The rate depends on treatment volume, payer mix and the services you need, and a billing specialist quotes it after reviewing your numbers.
Yes. We bill 90951-90970 by the patient's age group and the face-to-face visits documented that month, and we hold the 4-visit code until the fourth visit is in the record.
We confirm each patient's employer coverage and Medicare entitlement date, bill the employer plan first for the 30-month coordination period, and switch Medicare to primary when the period ends.
Yes. We work old dialysis AR by denial reason and payer, and across our clients we have reduced days in A/R from 54 to 36 with a 97.7% net collections rate.
We bill each treatment at the bundled rate with every case-mix adjuster the record supports, and keep ESRD-related drugs, labs and oral-only drugs inside the bundle. Services unrelated to ESRD go outside the bundle with modifier AY.
Yes. We bill home hemodialysis, peritoneal dialysis and training sessions with condition codes 74 and 73, and flag patients whose early home training moves their Medicare start date.
