Qualigenix medical billing services cover Cincinnati practices under the same Ohio rules we follow everywhere in the state, and those statewide rules are on our medical billing services in Ohio page. What changes in Cincinnati is the payer mix. Hamilton County residents apply for Medicaid through Hamilton County Job and Family Services, and most then join one of seven managed care plans. Patients on Traditional Medicare are billed through CGS, while many older patients carry Medicare Advantage or dual-eligible plans instead. Commercial claims here go to carriers such as Anthem, Humana, UnitedHealthcare, Medical Mutual of Ohio, Aetna and Cigna. Our billers work inside your existing EHR and send each claim by that payer's rules.
Qualigenix runs the full claim cycle for Cincinnati practices: eligibility checks, charge review, claim scrubbing, submission, payment posting, A/R follow-up and denial appeals. We bill all payers, including the seven Ohio Medicaid managed care plans that cover Hamilton County, the Next Generation MyCare plans, CGS for Traditional Medicare, and commercial carriers. You pay a percentage of collections, so our fee moves with what we collect.
CGS for Traditional Medicare, and commercial carriers. You pay a percentage of collections, so our fee moves with what we collect.
Most Cincinnati Medicaid revenue runs through managed care, so Qualigenix bills each plan as its own payer with its own portal, edits and payment timing.
Hamilton County members can join any of Ohio Medicaid's seven managed care plans: AmeriHealth Caritas Ohio, Anthem Blue Cross and Blue Shield, Buckeye Health Plan, CareSource, Humana Healthy Horizons in Ohio, Molina Healthcare of Ohio and UnitedHealthcare Community Plan. Members can change plans, so Qualigenix checks the plan on file for the date of service before the claim is built. Ohio Medicaid expects claims within 365 days of the date of service, and we track every claim from that date rather than from the date it reached us. Since June 2026, Ohio Medicaid has required every provider who treats managed care members to be enrolled and active in its Provider Network Management (PNM) system. Claims from billing or rendering providers who aren't enrolled, or aren't affiliated with the group on the date of service, are rejected at the EDI front door. We confirm both before your first claim goes out.
Two programs sit beside the main plans. Children and youth up to age 20 with complex behavioral health needs may be enrolled in OhioRISE, run by Aetna Better Health of Ohio. Their behavioral health claims go to Aetna, while physical health claims stay with their regular plan. Dual-eligible adults in Hamilton County moved into Next Generation MyCare on January 1, 2026, which pays Medicare and Medicaid benefits through one plan: Anthem, CareSource, Molina or Buckeye. Qualigenix reads the ID card and the eligibility response, then sends each claim to the plan that owes it.
Filing windows, Medicare contractor details and patient billing protections apply statewide, so we cover them in how Qualigenix handles Ohio Medicaid claims statewide. One recent statewide change shows up fast in Cincinnati: since October 6, 2026, Ohio Medicaid runs insurance discovery checks before it pays a claim. We capture every secondary policy at intake so those checks don't stall payment.
These are company-wide figures across Qualigenix client practices, not Cincinnati-only results.

Yes. We bill all seven Ohio Medicaid managed care plans that cover Hamilton County, plus OhioRISE through Aetna Better Health of Ohio. We confirm the member's plan and your PNM enrollment before each claim goes out.
Dual-eligible members in Hamilton County moved into Next Generation MyCare on January 1, 2026. Qualigenix identifies the member's MyCare plan from the ID card and eligibility response, then bills that plan for both the Medicare and Medicaid portions.
Ohio is one of six WISeR states, so certain Traditional Medicare services now need prior authorization or face prepayment review. Qualigenix flags those services at scheduling and submits the request before the visit, so claims aren't held for review.
Qualigenix charges a percentage of collections, so our fee rises and falls with what we collect for you. The exact percentage depends on your specialty, claim volume and services in scope. A billing specialist can walk you through it.
