Why is my clean claim rate below 95%?

A clean claim rate below 95% almost always comes from front-end eligibility and registration errors, charge-entry or coding mistakes, or claims submitted without payer-specific scrubbing. These happen before the claim reaches the payer, which is why they're fixable. A free billing audit isolates which one is costing you most.

What is a good clean claim rate benchmark for a medical practice?

95% or higher is the working benchmark. Top-performing operations run higher. Qualigenix holds a 98.2% first-pass clean claim rate on first submission. Below 95% means more than one in twenty claims is being reworked.

How do I improve my first-pass claim acceptance rate?

Verify eligibility at every visit, add a charge-entry review before submission, scrub every claim against payer-specific edits, audit coding accuracy regularly, and track the rate by payer and by coder so you can see the source. Most practices start with a free AR review to find the biggest gap first.

Should I outsource billing to fix a low clean claim rate?

Outsourcing makes sense when your team is stretched, volume is rising, or the rate hasn't improved despite effort. A specialist partner applies scrubbing rules and specialty coders already running at benchmark, so the rate climbs without new hires. Qualigenix onboards a practice in 6 days on average.

What does a medical billing company do to raise clean claim rates?

It verifies eligibility and charge capture on the front end, scrubs every claim against payer rules before submission, uses specialty coders for accurate CPT/ICD pairing, and works denials for root cause. Qualigenix runs this at a 98.2% first-pass clean claim rate and a 4.8% denial rate.

How long does it take to improve a clean claim rate?

In-house, expect months of process change to move the number. With a partner already operating at benchmark, improvement starts as claims move onto the new workflow. Qualigenix's average onboarding is 6 days.
